Here’s how pizza certificates will work:
We will be provide the certificates to ONE agency representative of your choosing, via email.
Your agency representative will be in charge of distributing the certificates to the house managers or other representative of your choosing at each group home location.
- We will NOT be requesting the addresses of any of the group homes.
- We are NOT responsible for the distribution of the certificates once we have provided the certificates to your agency representative.
- We reserve the right to distribute the gift certificates as we determine equitable and fair.
Here’s how holiday cards will work
The community will be provided the address or addresses you provide on this form! Do not provide any addresses that are private!
Please make sure there is an agency representative in charge of checking for cards at the address(es) you provide, who can distribute & deliver them!
